Cancellation Policy
We require a 24-hour notice ahead of your scheduled appointment time in order to make a change or cancel. Patients who do not attend a scheduled appointment or do not provide the proper notice to change a scheduled appointment will be responsible for a $75.00 office visit charge. This charge cannot be billed to insurance and must be paid on or before the next scheduled appointment. (Monday appointments need to be canceled by 12pm noon on the Friday before)
